If you would like your personal data to be removed from Digostics’ system.

How to Make a Request

To request removal of your data, please complete our Data Policy Concerns Form.

You will be asked to provide:

• Your Test ID

• OR your postcode and phone number (if you do not have your Test ID)

This allows us to locate your record and process your request securely.

Using the Form

When completing the form, please select:

• I have a question about test communications or use of my personal data

• I have questions about how you handle my personal data

What Happens Next

A member of the Customer Care team will review your request and may carry out a short security check before any changes are made. Your request will then be forwarded to your healthcare professional, as the NHS holds your data whereas Digostics merely processes it.
