How to Request a Change of Other Information
For any other updates, please complete our Update Contact Details form.
You will be asked to provide:
Your Test ID and one other identifier that is not being changed
OR two identifiers that are not being changed
Please select "Other"in the "Select the detail you would like to update" dropdown available in the form. Select "Other" and let us know what other detail you would like to update, and what you wish to update it to.
